JAMB Admission Letter: The official institution copy printed from the JAMB CAPS portal.
LASU Provisional Admission Letter: Printed from the LASU admission portal.

Anti Cult Oath Form: This is now a mandatory prerequisite for course registration. It must be completed, certified by a Commissioner for Oaths, and uploaded to the portal.

Certificate of Origin: For Lagos State indigenes, this is the IIVC clearance certificate. Non indigenes require a Local Government identification letter from their state of origin.
